Hospice Programs

Bereavement: Traditional Home Health and Hospice provides support for the family members through this difficult time. Our bereavement program will make contact with you over the next year following your loved one’s death. Grief Support Series: This is a weekly series that's offered in the Spring/Fall providing discussion for anyone grieving the loss of a loved one. Sessions are informal and take place at the office of Traditional Home Health and Hospice. Participants will be invited to listen and share their own stories as they seek personal healing and consolation.

Yoga for Grief: Yoga for Grief by Mindy Hill creates a safe and sacred space to process and explore feelings, thoughts and emotions as a result of grieving. Hatha yoga postures are offered to nurture the heart and relieve the physical manifestations of grieving. Breathing techniques, guided meditation and relaxation are taught to deal with grieving and loss. This group is designed for all levels. No experience in yoga in necessary.

Complementary Therapy Programs: These include Music Therapy, Art Therapy, Massage Therapy, and Pet Therapy, and are offered to our hospice patients. While it is certain that the clinical and spiritual components of our Hospice Program are exceptional, the addition of these complimentary therapies allows “moments of life” experiences through our patient centered treatment plans.

Continuous Care Program: This is compassionate, professional patient-centered hospice and palliative care for patients and their families facing an exacerbation of symptoms of a life-limiting illness. Using primarily nursing care, continuous care is tailored to meet the patient’s individual needs from 8-24 hours a day, with the presence of a qualified need.

Connections Program: This is a free, non-medical follow up program offered to individuals who do not qualify for home health or Hospice services; however, still need a “check-in” once in a while. Connections is our way of keeping connected with you, while keeping you connected to the community resources you need to safely stay at home.
